Pain that won't let you sleep
If hip pain wakes you up at night or prevents you from sleeping, this is a significant quality-of-life indicator. Rest pain indicates advanced joint damage.
Pain despite maximum medication
If anti-inflammatory medication, physiotherapy, and injections no longer control your pain, conservative management has reached its limits.
Cannot walk more than 100 metres
If you stop due to hip pain after walking less than one city block, your function is severely compromised. This is a surgical indication.
Trouble with daily activities
Difficulty dressing, getting in/out of a car, using the toilet, or climbing stairs due to hip pain — surgery restores these basic functions.
Limping significantly
A pronounced limp that affects your posture, back, and opposite knee is a sign the joint is failing. Prolonged limping causes secondary problems.
Hip stiffness throughout the day
Severe stiffness that limits your range of motion to less than 90 degrees of flexion makes surgery a reasonable consideration.
Bone-on-bone on X-ray
When X-ray or MRI shows complete cartilage loss (bone-on-bone contact), there is no conservative option that can regenerate cartilage. Surgery is the only lasting solution.
Grade 4 arthritis diagnosed
Radiological Grade 4 osteoarthritis means severe joint damage. Pain management alone is insufficient long-term.
Failed previous treatments
If physiotherapy, weight loss, walking aids, anti-inflammatories, and injections have all been tried without lasting benefit — surgery is the next step.
Quality of life severely affected
If hip pain prevents you from socialising, working, exercising, or doing things you love — this is a valid reason to consider hip replacement.

Myths About Hip Replacement Surgery
Myth: I should wait until the pain becomes unbearable
Fact: Waiting too long can lead to muscle weakness, bone loss, and increased surgical complexity. Earlier surgery (when symptoms significantly affect quality of life) gives better outcomes.
Myth: I'm too old for hip replacement
Fact: Age alone is not a contraindication. Patients in their 80s and 90s routinely have successful hip replacements. Overall health and fitness matter more than age.
Myth: Hip replacement will just fail again in a few years
Fact: Modern ceramic and metal implants last 20–25 years or more. With robotic placement and correct technique, revision rates within 15 years are below 5%.
Myth: I won't be able to do anything after hip replacement
Fact: The opposite is true. Most patients return to walking, swimming, cycling, gardening, golf, and travel. High-impact sports (running, jumping) are discouraged.
